Beginning 2025-2026, all physicals will need to be dated after April 1.  Per GHSA, any physical examination taken on or after April 1 will be good until the last day of the following school year.  Contact your location school athletic office for guidance.

A new Registration is required for each school year (you may not simply edit the one from last year).  Click the Student Registration link, in the upper right corner of the screen, to begin an athletic registration for your 8th - 12th grade student.  When uploading the signed documents, please make sure to upload the Physical Examination Form, Medical Eligibility Form, Cardiac Awareness Form, and Concussion Awareness Form.
